The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.


The Behavioral Health Clinician will work at Ft Lyons directly serving patients  in our Supportive Residential Community. The BHC will provide assessments and related diagnoses, therapy, education and consultation regarding behavioral health issues.


NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date.  The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance.  Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions 
  • Provide excellent internal and external customer service, fostering a team-oriented atmosphere with colleagues, partners, and clients. ]
  • Deliver comprehensive clinical services to homeless individuals with psychiatric diagnoses, including severe and persistent mental illness, and potential co-occurring substance-related disorders. This includes trauma-informed case management, screening, intake, individual service planning, progress documentation, service tracking, outcome evaluation, and group support for interested participants in a timely, organized, and accurate manner. 
  • Conduct psychosocial assessments and intake screenings with appropriate diagnoses.
  • Develop individualized treatment plans for clients that address their specific needs.
  • Monitor, evaluate, and update these plans regularly to ensure their effectiveness. 
  • Facilitate therapeutic meetings with clients in both individual and group settings. 
  • Work collaboratively as part of a multi-disciplinary team, adapting roles as needed based on the client’s team affiliation and care plan. 
  • Assist clients in identifying and accessing goods and services for both immediate and long-term needs, including identification, income maintenance benefits, medications, transitional and permanent housing, insurance benefits, and comprehensive health, mental health, and substance treatment services. 
  • Maintain accurate electronic health records (EHR), participate in audits, and undertake projects to ensure records meet standards of care.
  • Document all clinical services, telephone communications, and other interactions in a timely and complete manner.
  • Manage tasks and ensure all client charts are current.
  • Record client insurance status and complete necessary clinical authorization documentation.
  • Utilize the EHR and any future upgrades as directed. Facilitate client connections with other departments and service agencies.
  • Collaborate and coordinate with internal and external programs to facilitate client referrals and ensure continuity of care.
  • Establish and maintain effective working relationships with organizations providing services to our target population. 
  • Serve as a liaison with collaborating agencies and internal teams, maintaining effective working relationships and ensuring service continuity.
  • Availability by cell phone during regular business hours is required. 
  • The BHP will also assist patients in navigating the Coalition and community health care systems for optimal care. 
  • Participate in program outcome evaluations, audits, and other data reporting requirements to improve service quality and effectiveness.
  •  Conduct welfare and safety checks as needed, including implementing M-1 Holds when appropriate. 
  • Understand and maintain client confidentiality and adhere to HIPAA privacy practices.
  •  Connect clients to the integrated health team, including medical, psychiatric support, groups, dental, vision, and pharmacy services. 


Qualifications Summary 
  • Master’s degree in social work, counseling, or similar field; licensed LCSW or LPC with a CAC II or higher strongly preferred. If not licensed, must already be registered with DORA as a candidate for licensure (i.e. LSW,LPCC) and within 6 months of obtaining LCSW or LPC license.
  • Valid drivers license required. 
  • Previous working or volunteering experience with the homeless population preferred or experience in a related direct human service field working with underserved populations. 


$70,000 - $85,000 a year
WHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVENT YEARS OF EXPERIENCE
