Community Care Partner (Memphis)

We’re looking for a passionate and dedicated Community Care Partner to join our team and help drive meaningful change in the health of our TennCare members—particularly in Memphis, where the need is greatest. In this role, you’ll lead the execution of our Annual Outreach Plan , supporting more than 100 targeted outreach events each year. From maternity and EPSDT initiatives to school partnerships, focus groups, and food access programs, your efforts will directly impact health equity and ensure we meet regulatory and community-driven goals. You’ll build strong relationships with local providers, community agencies, and schools while coordinating educational and screening events, tracking outreach data, and supporting our volunteer team (Team Blue).

Our ideal candidate is highly organized, collaborative, and committed to serving underserved communities. Evening and weekend work will be required to support community schedules. If you’re ready to make a lasting impact through strategic, boots-on-the-ground outreach, we’d love to hear from you.

While this is a fully remote position, you will be required to travel in the Memphis/ West Tennessee area on a regular basis.

Job Responsibilities

  • Support the implementation of community-based population health activities ensuring that performance and timeliness goals are met and accurately reported, ensuring successful completion of assigned projects and monitoring the processes to ensure compliance with quality level and regulations and to reduce the business or fraud risk exposed to company.
  • Partners with community organizations and targeted providers to implement preventive health campaigns, screening health campaigns and educational programs, enhance member knowledge, and ultimately impact health outcomes. These include: health/safety presentations; face-to-face education and outreach sessions/workshops for members; culturally and linguistically appropriate health promotion/education programs.
  • Develops and maintains relationships with new and established community organizations. Acts as key player in outreaching to and penetrating new organizations.
  • Collaborates with internal departments and outside agencies to obtain data that is pertinent to the overall success of health education/promotion programs; makes appropriate member referrals for coordination of care/services by handling basic referral tasks and collaborating with care management teams, community-based health care providers, and other organizations.
  • Ability to travel, with short notice which may include overnight stays, up to 50% of the time
  • Ability to lift 30 40 lbs
  • Good driving record, background check may be required
  • Various immunizations and/or associated medical tests may be required for this position.

Job Qualifications

Education

  • Bachelors Degree in a Health Related Discipline (Nursing, Dietetics, Exercise Science, Counseling, Social work, or Health Education)

Experience

  • 3 years - Project management, account management or similar experience required
  • 3 years - Experience in developing, planning and implementing, comprehensive prevention, education and outreach initiatives, and coordinating community events which incorporate preventive screenings

Skills\Certifications

  • Proficient in MicroSoft Office (Outlook, Word, Excel, Access and Powerpoint)
  • Ability to work independently under general supervision and collaboratively as part of a team in a fast paced environment
