The Physical Therapist will focus on coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home. Alongside the Primary Care Physician, rehabilitation team, and other IDT members, the Physical Therapist works to maximize participant independence safety, and develop or restore function.

As a Physical Therapist, you'll play a crucial role in delivering top-notch restorative and rehabilitative therapy services, both at our center and in the comfort of participants' homes. Your mission will be to collaborate with the Primary Care Physician and an interdisciplinary team to enhance each participant's independence and safety, while also focusing on developing or restoring vital functions. Your day-to-day will include conducting assessments to identify individual care needs and preferences, actively participating in team meetings to craft personalized care plans, and implementing documented therapy sessions as outlined by physicians. You'll have the chance to guide therapy assistants and instruct participants and their families in using therapeutic equipment. If you have a Bachelor’s Degree in Physical Therapy and at least five years of experience, or a Master’s/Doctorate in Physical Therapy, we’d love to hear from you! A valid California Physical Therapy License is required, along with a passion for assisting the frail or elderly population.
