Responsibilities

Responsible for providing support and expertise through a continuum of care from admission through discharge. Supports the physician and interdisciplinary team to facilitate patient care, while enhancing the quality of clinical outcomes, patient satisfaction and managing the cost of care. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. Through close collaboration with the CRMC Command Center, the RN Case Mgr incorporates admission status and medical necessity determination, to ensure that appropriate care and services are delivered efficiently and timely. 

Qualifications

Education• Bachelor's Degree in Nursing required  Experience• 1 year of acute care hospital experience required• 3 years of acute care hospital experience may be substituted in lieu of Bachelor's Degree and experience requirement  Licenses and Certifications• RN - Current and valid Registered Nurse license to work within the state of California required• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
