Pay:  $50,000 Annual Salary

Schedule: Monday - Friday, Full-time, Remote position 

Supervisor title: Sr. Continuing Care Manager

 

General Summary: This individual plays a vital role in supporting individuals transitioning through different levels of Embark's care continuum, guiding them toward long-term stability and well-being. They provide essential resources, advocacy, and support to help clients navigate their journey through the different levels of care and programming based on an individual’s changing needs, from virtual counseling to residential services.

Essential Duties and Responsibilities:

  • Follow continuing care best practices and coordinate a seamless transition between programs and referral partners using checklists and tracking of caseload.

Qualifications

  • At least one year experience in the behavioral health industry either in a case manager, care coordination or outreach/intake capacity.
  • A desire to facilitate client growth through a team approach, working with other care providers.
  • Ability to develop trusting relationships with families, our program leaders and referral partners.
  • High attention to detail.
  • Technology savvy, with experience with video technology and telehealth platforms.
  • Ability to work with various referral partners including case managers, educational consultants, and mental health practitioners.
  • Preferred – Previous experience working with adolescents, young adults and their families in a clinical outreach or marketing capacity.
  • Preferred - A Bachelor’s degree in mental health or related field.
  • Preferred – prior experience in outreach and marketing.
  • Preferred – knowledge of how insurance works and talking to families about their insurance coverage
  • Preferred – experience using Salesforce. Knowledgeable on how to navigate EMRs.
  • Ability to pass a state background check.
