Otterbein SeniorLife is seeking an experienced MDS Nurse Transition Leader to oversee MDS duties across multiple locations while leading special projects.

  • Complete the MDS and Care Plans effectively
  • Discuss elder observations/concerns with appropriate coordinators
  • Generate/update Monthly MDS Calendar and Notification Process
  • Provide coaching and direction to staff
  • Facilitate discharge planning for residents
  • Act as the therapy liaison
  • Participate in weekly Medicare/therapy Utilization Review Meetings
