Our Clinical Care Specialists provide direct care and education to patients with chronic cardiometabolic conditions in collaboration with our Physicians. In the spirit of transparency, we anticipate hiring for this position in Q4.
Provide comprehensive diabetes, hypertension, hyperlipidemia and weight loss care, education, and support for patients through chat, phone, and video calls following internal protocols
Create thorough consults aligned with internal protocols for 9amHealth physicians
Collaborate with Patient Care Advocates to ensure patient success
Minimum Qualifications
Registered Dietitian Nutritionist plus Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) or eligible for certification
Experience in medication adjustment and using an EMR
Strong empathy skills and an expert in patient-centered language and care
A quiet, distraction free dedicated work space
Preferred Qualifications
Experience with telehealth or using technology platforms in patient care
