Sr. Supportive Housing Case Manager
Hours: 40 hours flex M-F, Rotating Weekends, Rotating late nights
Salary $22.50 Hourly
The Supported Housing Case Management program provides housing for chronically homeless single adults and families who are eligible for DSS benefits, SSI or SSD, with Behavioral health and Chemical dependency issues. It also provides housing for individuals who reside in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs and adult homes as well as housing for individuals who are and have been residents of either OMH Psychiatric Centers or OMH and OCFS operated residential programs for one year or longer.
The ideal candidate for this position will be skilled to:
•Engage the recipients and create a shared Housing Health and Recovery Plan that will reflect the principles of person-centered, strength-based recovery, with the ultimate goal of self-sufficiency.
•Provide outreach and assistance securing apartments in the affordable housing complex.
•Provide services that will touch all of the life domain areas including employment, finance, permanent relationships, medical needs, health, legal concerns, family, parenting, education, transportation, safety, culture, spirituality, and social.
•Partner with recipients as they integrate into the community of their choice and support them to make informed choices of the available supports, services, and recreational opportunities.
•Assist with a plan for discharge when the recipient is ready to transition out of Supported Housing. Anticipate supports and services needed to highly support the transition and ensure implementation of all linkages to facilitate success.
•Serve as a model, guide to other staff on the SHCM team.
•Respond and Intervene in crisis situations as needed
• Complete required documentation of services provided and assist in orientation of other staff as needed
•Comply with regulations set by employer, as well as NYS OMH
•Work closely with community providers on facilitating a smooth transition for all referrals into the Supported Housing Program.
•To work a flexible schedule evenings and weekends as needed to ensure the needs of all recipients are met.
•Off-hours on-call and emergency situation response
Qualifications:
2 years of experience working in case management or mental health services and/or equivalent experience
•Excellent interpersonal and engagement skills
•Very Good organizational skills
Education:
BA required, minimum 1 year exp.
