If you’re eager to make a tangible difference in people’s lives, to help correct long-standing disparities in health care, join us.About the role:As a Patient Experience Navigator (PEN) for Accompany Health you will:-Be an advocate for our patients who are often overlooked in our healthcare system-Support our organizational success metrics by improving and maintaining patient ongoing engagement rates and helping us meeting your quality goals-Be an early member of our collaborative team providing care coordination to populations with complex healthcare needsResponsibilities will include:• Manage follow-up appointment scheduling for patients• Help patients schedule and coordinate referrals, imaging, procedures, and screenings appointments and support them in completing these visits• Process prior authorization and DME requests• Organize and locate patient medical records• Support a care pod with daily operational activities including visit planning, virtual visit troubleshooting, and assisting patients with urgent needs• Collaborate with your care pod to carry out patient care plans• Maintain patient confidentiality and abide by HIPAA standards• Other duties as assigned• This role reports to the Manager of Market Operations• Working hours are Monday-Friday 8:00am-5:00pmWhat makes you a fit for the team:• You are excited to work in a startup environment, with the ambiguity and shifting priorities that might come with it at times.• You are willing to go the extra mile no matter what.• You are passionate about our mission to improve the lives and healthcare outcomes of marginalized communities.Desired skills and experience:• Required• At least 2 years of patient-facing experience in a healthcare setting• At least 1 year of scheduling experience with orders including DME• Previous experience with EMR systems (Athena knowledge is a plus!)• Excellent customer service skills• Ability to quickly build trust with patients• Strong communication skills, and a flexible and positive attitude• Strong computer skills and basic knowledge of G-Suite (e.g., Google Docs)• US work authorization• Preferred• Medical Assistant CertificationFor Patient Facing RolesTo keep our patients, communities and each other safe, you'll be required to comply with Accompany Health’s medical clearance requirements, including completing a TB screen and providing proof of immunity or vaccination for certain conditions.
