About the role:


Urgent Care Advanced Practice Clinicians are a key part of our care model, which also include Physicians, Advanced Practice Clinicians, Community Health Workers, Patient Experience Navigators, RNs, Social Workers, Behavioral Health Clinicians, Psychiatrists, and Pharmacists. Together this team is responsible for providing and coordinating holistic, patient-centered care for an intimate panel of patients with complex medical, behavioral health, and social needs. 


As an Urgent Care Advanced Practice Clinicians, you will be part of the central urgent care team that will care for patients in multiple markets. You will care for patients primarily virtually via video, telephone, or text. As part of this team, you will work closely with a Clinical Triage RN who will help triage the clinical cases and escalate clinical issues to you. You will be responsible for providing urgent care for the patient, and responding with compassion and empathy to ensure that patients are connected to appropriate care and resources that can keep them safely at home and out of the hospital when possible. 


Responsibilities will include:
  • Perform clinical triage for inbound outreach from our patients to ensure the highest level of patient experience, safety, and connection to appropriate resources that can keep them safely at home and out of the emergency department/hospital when possible.
  • Provide urgent care for patients primarily virtually via video,  telephone,  or text  by providing virtual assessments of clinical symptoms, urgent clinical care, and supervise close care coordination.
  • Collaborate closely with local Accompany Health teams such as Advanced Practice Clinicians and physicians as well as external providers to ensure appropriate resolution of triage issues. 
  • Effectively interpret and utilize electronic data tools and analysis to organize daily activities and provide high quality of care
  • Establish and foster trusting relationships with your patients and ensuring that care is appropriately aligned with their goals and values
  • Provide feedback on program design and workflows to ensure we are providing the best patient care possible.
  • Ensure timely and appropriate documentation.
  • Roles and responsibilities may evolve as our care model develops. 
  • Occasional in person team building time

Desired skills and experience:
  • Required
  • Active Physician Assistant or Nurse Practitioner license in home state and willingness and certification in good standing and the ability to get licensed in requested states such as Michigan, Colorado, Indiana within 90 days of hire date
  • NCCPA board certification for Physician Assistants, AANC or AANP board certification for Nurse Practitioners
  • Active DEA Controlled Substance Registration or eligible for application. X-waiver licensure (for buprenorphine prescribing) is a plus but not required
  • Active controlled substance license 
  • Certified in Basic Life Support for Healthcare Providers
  • 3+ years of clinical experience in providing care in internal medicine, family medicine, emergency medicine, or urgent care
  • Possess exceptional clinical triage, and clinical assessment and urgent care skills
  • Demonstrated ability to help a patient adapt new habits, change behaviors, and motivate towards achieving health goals. 
  • Comfort with electronic medical record documentation and excited about how technology can support your work and drive ongoing improvement towards new and better care
  • Experience and comfort working within an interdisciplinary care team, and specifically communicating with clinical and non-clinical team members

  • Preferred 
  • Experience providing care in virtual settings
  • Experience in clinical triage and urgent care.  
  • Experience and passion for caring for individuals with complex chronic medical, behavioral and social needs
  • Experience  with engaging individuals with untreated and/or symptomatic chronic mental illness and addiction
  • Experience in trauma-informed care and practices. 
  • Experience as an active participant in continuous quality improvement projects.
  • Experience in value-based care organizations


For Patient Facing Roles

To keep our patients, communities and each other safe, you'll be required to comply with Accompany Health’s medical clearance requirements, including completing a TB screen and providing proof of immunity or vaccination for certain conditions. You will also be required to be vaccinated and up-to-date on your COVID-19 vaccinations, including boosters. This is a condition of employment, and we make exceptions as required by law. Accommodation for religious and medical beliefs will be provided on a case by case basis.


For Non-Patient Facing Roles

To keep our patients, communities and each other safe, you'll be required to comply with Accompany Health’s medical clearance requirements, including receiving the COVID-19 vaccine. This is a condition of employment, and we make exceptions as required by law. Accommodation for religious and medical beliefs will be provided on a case by case basis.
