Team Lead - Employment Services

As a Team Lead in our Employment Services team, you'll act as a coach and mentor to the specialists on the team. You will focus on assisting Employment Specialists with caseload management, team morale, productivity and problem solving on an ongoing basis. You'll act as a content specialist, be a superuser of our EHR, and lead some of the team processes and procedures including the Designated Coordinator process, coordinating shadow visits, case documentation and other checklists and procedures.

Essential Responsibilities

  • Listen-Learn-Lead: Model the mission and values of Accord, helping people to live their greatest lives.
  • Provide flexible, individualized employment supports, offering employment services in all employment service areas (waiver and VRS) 
  • Provide 1:1 coaching and guidance to team members on caseload management, team morale, productivity and problem solving on an ongoing basis. 
  • Mentor, coach, train, and onboarding of new hires including coordination of shadowing opportunities. 
  • Collaborate closely with the team Manager to provide updates on team members' performance.
  • Conduct or shadow intakes for new team members. 
  • Audit documentation of team members for compliance and quality
  • Complete Designated Coordinator reviews for the Employment team including in-person site visits. 
  • Serve as the content expert for Employment services. Respond to questions from outside stakeholders.
  • Assist with content and engagement on team Resource and Bulletin board G-chat channels and monthly team meetings.  
  • Review, contribute, and uphold processes, checklists, and procedures. 
  • Become a superuser with the Electronic Health Record (EHR)
  • Attend all required annual and other meetings, demonstrating a high level of professional oral communication and genuine care for the person supported.
  • Demonstrate strong written communication skills; complete all required paperwork to comply with licensing and funding sources such as annual reviews.
  • Complete documentation of all services provided in the selected electronic health record/database system within 1 business day of providing services.
  • Meet or exceed minimum billing standards of 70%
  • As a mandated reporter, respond and report all incidents that occur while providing services to protect the health and safety of and minimize risk of harm to the person(s) supported following Accord’s policy and MN Statutes section 245D.02, subdivision 11.
  • Demonstrate professional communication with interdisciplinary team members including the person supported, family caregivers and guardians, employers, case managers, and other involved stakeholders.
  • Provide transportation as needed to individuals supported    
  • Complete all required and assigned training related to the position
  • Travel throughout the Twin Cities Metropolitan area to job sites and meeting locations; limited overnight travel possible.
  • Perform other duties as required or assigned.

Education:

  • Meet one of the qualifications listed below and provide documentation of them including a copy of the degree earned  and your transcript showing the date awarded and the degree earned and a resume which documents the year(s) of direct care experience and populations supported. 
    • A four-year bachelor’s degree in a field related to human services and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR 
    • An associate degree in a field related to human services and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
    • A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
    • A minimum of 50 hours of education and training related to human services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualification identified above. 

If you hold a Social Work degree, we expect you to be licensed by the Minnesota Board of Social Work, as required. 

Skills & Experience:

  • Demonstrate competency by holding an industry-recognized credential from the Association of Community Rehabilitation Educators (ACRE) with CE designation through VRS. 
  • Excellent time management skills
