How Is This Role Important to Our Work?
Provides general medical care and treatment to patients in a mobile and virtual, team-focused setting. This role will be responsible for accurate patient histories, symptoms, and physical findings to develop diagnoses and treatment plans; order and perform relevant diagnostic tests; prescribe medication; and ultimately develop treatment plans based on evidence-based practice, standards of care, and professional practice guidelines. The role will also educate patients on self-management of acute or chronic illnesses and recommend interventions to modify behavior associated with health risks. This role with partner closely with other clinicians, case managers, care coordinators, social workers, therapists, pharmacists, and other team members to ensure patients are receiving holistic, humane care.
Overview
Livio Nurse Practitioners spearhead Livio’s clinical mission—to provide exceptional, humane, equitable care to medically complex patients with serious illnesses. In collaboration with Livio’s interdisciplinary team, nurse practitioners will design and implement care plans that enable patients to achieve their life goals while confronting serious health challenges. Nurse practitioners will provide direct care to patients in their homes and will use telephonic and telemedicine resources to monitor patients’ symptoms and emotional well-being. This is a unique opportunity—a chance to innovate and improve the quality of care for patients and their loved ones.
A Day in the Life:

Perform comprehensive in-home assessments of patients with advanced illness, including assessment of medical, mood, functional and cognitive status, as well as psychosocial and family support.

Build personalized care plans that tailor medical interventions to patient goals and clinical status.

Complete medication reconciliations and guide thoughtful prescribing and deprescribing, in partnership with patients’ other providers.

Discuss and define goals of care with patients, families and caregivers facing advanced illness.

Collaborate closely with colleagues in care coordination and social work to deliver person-centered care that enables patients to achieve their life goals.

Work collaboratively with community partners and specialists to craft individualized care plans for patients with advanced illness.

Create plans to address patients unmet social needs, including addressing barriers to equitable care and involvement of community based organizations as needed

Work collaboratively with area hospice and palliative care providers to provide smooth transitions to end of life care for qualifying patients.

Participate as facilitator/teacher in Livio’s ongoing internal clinical education.

Participate in ongoing quality improvement efforts as Stella refines its model of care.

Use telemedicine resources to provide clinical care, as appropriate.
