Health Connections Assistant VISTA

Join us to gain valuable experience in the field of community health and contribute to positive change alongside a supportive team at Henry J. Austin Health Center (HJAHC). At HJAHC, we provide trauma-informed, holistic care to maximize individuals’ strengths and abilities to achieve optimal health and well-being and strive to eliminate health disparities in Mercer County, New Jersey. Our VISTA members will receive personalized mentoring and professional development opportunities. The Health Connections Assistant VISTA will focus on improving the social determinants of health and creating health equity for Henry J. Austin Health Center patients. The VISTA member will play an integral role in launching a new volunteer program in which Patient Advocates receive specialized training in delivering community resource referrals and social needs interventions for patients. The Health Connections Assistant VISTA will recruit and facilitate training of volunteers, research community resources and troubleshoot barriers to access, and monitor and evaluate the impact of this pilot program in addressing patient needs and Social Determinants of Health. Through these activities, the VISTA will strengthen HJAHC's methods for patient care, amplify Henry J. Austin Health Center’s impact, and foster meaningful connections with patients and the community.Further help on this page can be found by clicking here .Member Duties : Under the direction of the Development & Community Engagement Manager, the Health Connections Assistant (AmeriCorps VISTA) aims to improve patient access to community resources, address social determinants of health, and enhance the overall well-being of HJAHC patients. The VISTA member will collaborate with various stakeholders to implement and evaluate the impact of the Health Connections pilot volunteer program, identify and address barriers to patient access.
