About our Team:

We employ a field-based, home-based care model and are committed to meeting members where they are-in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.

About the Role:

In this role, you will provide administrative and operational support to the North Carolina Pregnancy Program Team. You will bring a task-driven, detail-oriented and collaborative approach to your work. You will interact with our members with warmth and compassion, helping to facilitate scheduling and connecting them to external resources and/or services as needed. You will report directly to the Pregnancy Program Clinical Lead with a dotted line to the Perinatal Care Team Lead.

You will:

  • Support administrative and operational functions of the North Carolina Pregnancy Program team, as an acting member of that care team and liaison to hub team.
  • Serve as a representative of Cityblock Health to members, other health care organizations, external practices, abortion clinics, insurance partners, and other partners in ensuring that our members have the care and assistance they need to thrive.
  • At the direction of the Pregnancy Program Clinical Lead, work closely with team members to coordinate care as a team on behalf of members.
  • Weekly tasks could include:
    • Schedule management for pregnancy program team members
    • Manage referral follow up and loop closure
    • Work with pregnancy program team members to coordinate social service assistance and transportation services for our members
    • Connect with insurance, external providers, and other organizations to request and deliver appropriate medical services and records for members
    • Support team meetings with note-taking and follow-ups on all commitments
    • Coordinate & communicate team calendar
    • Manage periodic certification and vendor requirements for pregnancy team staff
    • Use member benefits platforms to organize Medicaid support and enrollment
    • Manage logistics (rooms, food, planning) for case conferences, development sessions, meetings and team outings
    • Helping with special projects and aiding with company-wide initiatives and roll outs
