Rotating Weekends- 1 weekend a month required
Overview:

Birth Registrar

The Birth Registrar is responsible for accounting for all birth certificates on each occurrence of birth in the hospital. Responsible for obtaining information from parents to complete hospital worksheet and typing original birth certificates. Uses state provided software to process work. Also responsible for performing all other related tasks pertaining to the completion of the birth certificates and parental establishment processes. Knowledge of state regulations concerning Vital Statistics must be maintained to assure compliance.

Qualifications:
  • High School Diploma or equivalent preferred
  • Healthcare/Medical - Medical Records experience preferred
  • Clerical experience preferred
Responsibilities:
  • Prints on-line patient care documentation. Documentation is printed from appropriate system containing needed information.
  • Collects all required data elements, completes applicable forms; transfers information to the original certificate. Each data element is collected from the parent and patient chart to accurately and totally complete the original birth certificate.
  • Assists patient in the completion of hospital worksheet for each live birth. Explains VS-37 requirement when necessary. Worksheet is totally completed. Form VS-37 completed when applicable.
  • Provides Declaration of Paternity forms as needed. Declarations are distributed and prepared in a timely fashion following established procedures.
  • Manually types error-free certificate or enters error-free birth data into EBC software via PC prior to discharge. Error-free birth certificate completed prior to discharge.
  • Obtains parent's signature on each certificate on a timely basis to complete birth certificate.
  • Files hospital worksheets/certificate copies. Worksheets/certificates filed correctly.
  • Assists in answering and explaining information required on certificates when questioned by parent/family or physician, concerning the needed data and State Regulations. Answer questions as clearly and concisely as possible.
  • Prepares verification of birth for every patient. Verifications are prepared in a timely fashion following established procedures.
  • Updates newborn registration screen in Hospital computer system by adding the baby's name. The newborn's "baby boy/baby girl" registration status is changed to his/her name one day after discharge.
  • In the absence of the Lead Birth Registrar download and mail completed birth certificates to Vital Records in Richmond weekly; and complete AOP report on weekly basis (Tuesdays), completes final birth count for previous month and submits by 10th of each month to Vital Records.
  • Assists with completing fetal death certificates. Completed fetal death certificates are sent to Vital Records Department in Richmond. Fax Fetal Death reports on the 15th of each month.
  • Audits all clinic records for required documentation of delivery attending as required for Billing purposes. Documentation is submitted to Operations Manager.
  • Completes weekly productivity sheet. Accurate productivity sheet is submitted weekly.
