Our hospitals, care sites and clinicians are recognized for clinical and operational excellence.Primary Function/General Purpose of PositionThe RN Triage Specialist provides telephonic triage to assist callers to determine the most appropriate level of care needed for the current situation expressed or assessed, following workflows and utilizing protocols/resources to provide supportive service to patients and customers.The RN Triage Specialist will maintain a performance standard that prioritizes safety, quality and experience and coincides with the organization's mission and identified key strategic or performance initiates.Part Time - 24 Hours Per Week - 11pm-7:30am EST - Rotating weekends and holidays• Shift time subject to change based on need of operationsEssential Job Functions• Provides telephonic triage or requested support and / or virtual monitoring. Offers subsequent recommendations, education or care advice using decision making tools, clinical judgement, and defined workflows.• Participates in care coordination, by partnering with customers to reduce readmissions, enhance chronic disease management, manage health risk and injury reporting. Schedules provider appointments and facilitates provider communication.• Ensures accurate, timely documentation in the EMR (Electronic Medical Record) according to best practice, guidelines, or workflows.• Participates in virtual monitoring and subsequent reporting and escalation to support services identified by customer. Provides additional support to Conduit Health Partners business functions as identified to ensure all patient needs are being met and continuity of Conduit Health Partners business operations is maintained. • Participates in process improvement, professional development, peer development and peer reviewLicensing/CertificationRN license required in applicable state(s).Multistate/Compact RN Licensure preferredEducationADN or Diploma Nursing requiredBSN preferredWork Experience1 year of acute care nursing experience required.Triage experience preferred.TrainingEPIC Electronic Health Record (preferred)Working Conditions -Periods of high stress and fluctuating workloads may occur.General office environment.May have periods of constant interruptions.Required to car travel to off-site locations, occasionally in adverse weather conditions.Prolonged periods of working alone.Other: Remote/ At Home work Environment Opportunity may be provided.
