Provider Outreach Nurse Liaison

By The Bay Health, a non-profit since 1975, is dedicated to providing high-quality hospice and home health services. We are seeking an RN for the Provider Outreach Nurse Liaison role to foster relationships with healthcare referral sources.

Skills

Nursing experience in home health or hospice
Strong communication and interpersonal skills
Knowledge of hospice policies and regulations

Responsibilities

Develop relationships with referral sources
Determine patient eligibility for hospice care
Admit patients and obtain informed consents
Assist with hospital discharge coordination

Education

AA or B.S. degree in Nursing from an accredited institution
