As a Care Navigator at Sollis Health, you will work closely with the medical team to help coordinate all aspects of our patient's medical care. The healthcare system can be very difficult to navigate and with your help and advocacy, our patients will look to you for guidance and support every step of the way. This includes scheduling appointments with specialists, coordinating a hospital discharge, etc.
This role is Full Time and remote.
Responsibilities
- Working closely with patients to understand their needs and help to find them the best care based on their medical condition
- Coordinating physician referrals for patients who need to see specialists (i.e. cardiology, OBGYN, internal medicine, urology etc.) inside and outside of the Sollis medical partner's network
- Coordinating requests for private nursing, hospice, and other external care needs
- Collaborating with physicians, nurses, and other disciplines involved with care of the patient to foster a coordinated approach to care planning
- Preventing delays in patient care by communicating barriers to leadership for resolution and trending
- Researching and facilitating connections with new medical partners, healthcare facilities and services as needed
- Updating patient records in our Athena Health and CRM (Salesforce)
- Sending medical records as needed to our patients' physicians
- Providing a high-quality patient experience to all Sollis Health members
- Partnering with Sollis' sales and operations team to suggest process improvements and care coordination
- Performs related duties as required
Experience
We believe extraordinary people come from a variety of backgrounds, but ideally we would expect that you have:
- Associates degree required; Bachelor degree and above strongly preferred
- 2+ years of administrative, coordinator, front desk or related experience required
- Previously have worked in operations within an urgent care practice, concierge physician office, medical dept or hospital division strongly preferred
- Previous experience using a CRM, EHR and/or tracking database required

Range: $30-$35/hr
This is the anticipated rate/range Sollis Health reasonably expects to pay candidates for this position in New York City. Sollis is a multi-state employer and this rate/range may not reflect the pay for positions that are performed solely in localities outside of this location. Actual pay is dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, location, education, etc.
