The Housing Stability area of Hennepin County's Health and Human Services is seeking multiple Case Management Assistants to be part of the Homeless to Housing Program, a 30-person housing-focused case management team. These positions will have caseloads and will be working with people staying in shelters, outside, and in encampments to connect them to permanent housing by using a housing-first approach, relationship building, creating housing goal plans, and breaking down housing barriers.
Current openings are on the Youth and Young Adult Navigation Team (18 to 24 yrs.old) and on the Single Adult Long Term Homeless Team.

Location and hours:
These positions are hybrid and will require working at community sites within Hennepin County and remotely as job duties require. Work hours are typically Monday through Friday, 8am – 4:30pm, While these positions are designated as hybrid, based on current requirements hires must reside in or be willing to relocate to Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In these positions, you will:

Assist through all aspects of the housing search process including engaging with shelter guests to work through concerns and barriers and co-creating housing goal plans, assisting with applications, and gathering needed documents, as well as leading the housing search and move-in processes.

Be assigned clients from a centralized source and will have clients in different settings throughout the system and will need to be mobile and accompany clients to appointments as needed.

Learn relevant resources, systems (such as Coordinated Entry), and referral streams to connect clients to eligible benefits, make referrals to long-term case management services, and housing opportunities to stabilize people and assist in achieving housing goals.

Attend regular case consults and work closely with the full housing case management team.

Meet expectations around case noting, HMIS data input, and being data-informed in service delivery.

Be responsible for transporting and accompanying clients to various housing focused appointments as needed

One of the following:
-
Two years or more of responsible, paid social service experience providing direct client service.

A two-year human services certificate.

Two years or more of college coursework in the behavioral sciences.

A valid driver's license and access to reliable transportation for travel to various shelters and drop-in centers throughout Hennepin County.

Ability to use own vehicle on a frequent basis to transport clients to appointments and showings.
Nice to have:

Lived experience with housing instability.

Communicate effectively both written and verbally.

Communicate fluently in a second language in addition to English, both verbally and in writing.

Working knowledge of Homeless Management Information System (HMIS).

Knowledge of community resources and social services available to clients.
