The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.

Job Description

This position functions as the top executive for Laguna Honda Hospital and Rehabilitation Center (LHH) and is responsible and accountable for all functions and activities of LHH. This is a senior executive-level position and will report to the Director of the San Francisco Health Network. They lead, guide, and direct the operations of the healthcare facility in accordance with local, state, and federal regulations, standards and established facility policies and procedures to provide appropriate care and services to residents.

They also provide leadership and direction in developing new programs and establishing organizational policies, priorities, and objectives; directs and coordinates the implementation of program changes; serves as technical advisor; maintaining state and federal regulatory compliance; and performs related duties as required. They make decisions which directly impact the entire facility and inappropriate decisions may result in an irreversible impact on the health and welfare of the public. This position exercises supervision over LHH related functional areas.

Specifically, under general direction from the Director of the San Francisco Health Network, the LHH Nursing Home Administrator (NHA):

  1. Ensures delivery of compassionate quality care and services across an interdisciplinary team approach as evidenced by adequate, and competent facility staff, employee engagement, general cleanliness, physical plant condition, and optimal resident functioning-physically and psychosocially.
  2. Plans, develops, organizes, implements, evaluates, and directs the overall operation of the facility as well as its programs and activities, in accordance with guidelines issued by the governing body, current state and federal laws and regulations
  3. Identifies, in conjunction with the Director of Nursing and selected department heads, the facility’s key performance indicators. Establishes an ongoing system to monitor these key indicators such as the Quality Assurance and Performance Improvement (QAPI) process throughout the facility.
  4. Develops and implements processes and systems that maintain the fiscal health of the facility.
  5. Serves as the facility representative and spokesperson to the public as well as in the professional arenas.
  6. Leads and coordinates daily, weekly, bi-monthly, and/or monthly management team meetings to discuss priorities and develop solutions with facility leaders such as census, financial collections, clinical health, survey readiness, customer service satisfaction, activity participation, and resident satisfaction with a homelike environment, etc.
  7. Develops and maintains positive relationships with residents, employees, and family members through scheduled, deliberate positive interactions with them using face to face, phone, or other means of communication.
  8. Knows and understands general nursing practices and procedures, OBRA regulations, Code of Federal Regulations, Appendix PP State Operations Manual, reimbursement processes, Life Safety Code regulations, applicable labor relations laws, and all other regulatory entity requirements that may apply.
  9. Understands CMS program initiatives such as value-based purchasing programs like SNF-VBP, SNF-QRP, Quality Measures, Nursing Home Compare (also known as CMS Compare), Payroll Based Journal reporting, etc.
  10. Performs rounds to observe and meet residents and ensure overall needs are being met; models servant leader principals to regularly round and visit the patient care environment.
  11. Conducts periodic observations of in-service education to ensure staff members delivering the education are competent with the knowledge and skill set required to accomplish employee learning.
  12. Promotes teamwork, mutual respect, and effective communication. Promotes positivity and active daily problem solving.
  13. Leads, guides, and directs facility readiness related to health inspection surveys, Occupational Safety and Health Administration (OSHA) surveys, and any other regulatory entity or requirement.
  14. Ensures the facility’s plan of correction response to any regulatory, or inspection, survey is completed, adequate, implemented, and timely.
  15. Responds and resolves complaints and concerns when necessary
  16. Manages and minimizes facility risk through a team approach to achieve desired outcomes in customer service, key performance indicators, employee retention and other areas as identified.
  17. Ensures resident incidents and concerns that rise to a reportable event such as alleged abuse, neglect, mistreatment, misappropriation, etc. are reported to the correct entity within the stated regulatory requirement.
  18. Promotes and encourages an environment of trust among all employees related to the overarching goal of resident safety and abuse prevention.
  19. Promotes safe work practices, safety rules, and accident prevention procedures to prevent employee injury and illness.
  20. Delegates administrative authority, responsibility, and accountability to other staff personnel as necessary.

The LHH Nursing Home Administrator (0943 Manager VIII) may perform other duties as assigned or required.

Qualifications

License:

Possession of a current, valid, unrestricted license as a Nursing Home Administrator issued by the California Department of Public Health AND

Experience:

Seven (7) years of verifiable, full-time experience managing a licensed long-term care facility, all of which must include supervision of professionals.

Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.

One-year full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

DESIRABLE QUALIFICATIONS: The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring:

  • Active member of the California Association of Health Facilities (CAHF) and/or American College of Healthcare Executives (ACHE) 
  • Completion of ACHE or CAHF Leadership Training or equivalent training 
  • Seven years of experience as the NHA in a nursing home that has successfully achieved a Nursing Home Compare Star rating at 4 to 5 stars 
  • Success in certification or recertification of a Long-Term Care facility 
  • Successful executive-level management and leadership in a licensed long-term care facility managed by federal (such as VA), state, or local government
