Behavioral Health Specialist

Cityblock Health is seeking a Behavioral Health Specialist to join our dedicated team in providing care programming for underserved communities in the Chicago area. We are looking for individuals who are passionate about improving the health and well-being of our members.

Skills

Strong problem-solving skills
Understanding of mental health counseling
Experience in Motivational Interviewing and Cognitive Behavioral Therapy
Comfortable using technology for virtual care

Responsibilities

Participate in case conferences and care planning
Provide clinical education and support to members virtually or in-person
Facilitate behavioral health and substance use disorder referrals with internal and external providers
Manage behavioral health escalations and transitions of care
Utilize technology platforms for data collection and member communication

Education

Master’s Degree in Social Work or Mental Health Counseling
Active LICSW, LCSW, LISW, LMHC or equivalent licensure
