Behavioral Health Specialist - NC (Charlotte)

Cityblock Health is seeking a Behavioral Health Specialist to join our dedicated behavioral health care team. The role involves providing behavioral health services and care programming to our members, addressing the needs of members presenting with serious mental illness or substance use disorder, managing behavioral health escalations, and facilitating transitions of care as needed.

Skills

Master’s Degree in Social Work or Mental Health Counseling
Active LICSW, LCSW, LISW, LMHC or equivalent
Comfortable using technology for member support
Strong problem-solving skills
Versed in various therapies and care principles

Responsibilities

Participate in case review and care planning
Provide clinical education on preventative care topics
Support members' behavioral health needs virtually or in-person
Encourage access to evidenced-based treatments
Make referrals to care team if needed
Collaborate with external providers
Assist with behavioral health transitions of care
Utilize care facilitation platforms for data collection and communication

Education

Master’s Degree in Social Work or Mental Health Counseling
