Care Coordinator, Transitions of Care - IL (Chicago)

Cityblock Health is seeking a Transition of Care (TOC) Care Coordinator to help members transition from acute care settings back into the community. The role involves engaging members, conducting assessments, developing care plans, providing follow-up care, and collaborating with the TOC team.

Skills

Experience in community care or care coordination

Responsibilities

Reach out to schedule post-discharge visits for members
Complete assessments and assist in discharge planning

Provide routine non-clinical education and address member needs
Collaborate with internal and external providers for member care
