Provide comprehensive care management and chronic disease management
Perform urgent medical/behavioral health visits and post-discharge visits
Collaborate with specialists and community organizations for member care
Attend daily team huddles and contribute to clinical program development

Education

Certified in Basic Life Support for Healthcare Providers
Maintenance of professional credentialing and CME standards
