Skills

Board-Certified Nurse Practitioner or Physician Assistant
Experience in clinical services for chronic medical and behavioral health conditions
Proficient in clinical nursing skills like wound assessment and care
Strong independent clinical decision-making skills
Proficient in using EMRs

Responsibilities

Provide comprehensive care management and chronic disease management
Perform urgent medical/behavioral health visits for members
Conduct home visits for post-discharge care
Interface with specialists, hospitals, and community-based organizations for collaboration
Attend team meetings and case conferences

Education

Active license to practice in the state of Ohio
AANC or AANP certification for Nurse Practitioners
Certified in Basic Life Support for Healthcare Providers
Maintenance of professional credentialing and CME standards
