Active RN License
3+ years of experience
Tech-savvy with ability to use virtual platforms
Proficiency in documentation and data collection
Strong problem-solving skills

Responsibilities

Manage a panel of rising and high-intensity members
Collaborate with members to create care plans
Oversee progress and reassess needs
Coordinate with providers and specialized resources
Collaborate closely with Community Health Partner

Education

Graduate of an accredited school of nursing (R.N.)
Basic Life Support (BLS) Certification
