Skills

RN License
Basic Life Support (BLS) Certification
Experience in healthcare
Proficiency in using technology for remote support
Strong problem-solving skills

Responsibilities

Create care plans for members with chronic disease and behavioral health needs
Coordinate with providers and specialized resources
Collaborate with Community Health Partner for advocacy
Provide clinical support for a panel of members
Ensure quality care and proper documentation

Education

Graduate of an accredited school of nursing
