SSG Fox Program Intake and Assessment Coordinator

Clear Path for Veterans is seeking an Intake and Assessment Coordinator to deliver holistic programs for veterans, primarily focused on suicide prevention. The ideal candidate will be responsible for assessing veterans' eligibility for services and providing supportive referrals.

Skills

Excellent interpersonal and communication skills.
Working knowledge of HIPAA.
Ability to work with diverse individuals.

Strong organizational and prioritization skills.

Responsibilities

Assess Veterans for suicide risk using VA-approved clinical assessments.
Conduct warm hand-off to the VA for suicide prevention services.
Connect eligible Veterans to Peer Mentors for support.
Assist veterans experiencing crises during program-related activities.
Complete required administrative paperwork for program components.

Education

Bachelor's Degree in Social Work or Human Services required.
