MGA Homecare has been providing Private Duty Nursing, Skilled Nursing Visits, Occupational Therapy, Physical Therapy, and Speech Therapy services within the home and community-based setting for over a decade. In 2022 MGA has opened its doors to MGA Behavior Therapy which offers Applied Behavior Analysis (ABA) and early behavior intervention for pediatric patients with autism spectrum disorder (ASD) and other developmental disabilities within the Colorado Springs and surrounding areas.

Job Description


The RN Case Manager is responsible for providing and documenting skilled nursing care to pediatric and adult patients throughout their continuum of care and reports directly to the Clinical Supervisor. This position ensures compliance with MGA policies, state policies and procedures, and policies of accrediting agencies, as well as responsible for maintaining compliance with all reimbursement guidelines and maintaining patient confidentiality/HIPPA guidelines. The clinician will manage and follow each patient’s plan of care to meet the patient’s needs in accordance with physician orders.

Responsibilities/Duties

  • Maintains and manages a caseload of a minimum of 20+ visits weekly
    • Each visit reflects the provision of skilled nursing care rendered in accordance with physician orders assuring the client's needs are continually assessed and assuring the development, implementation, and updates of the individualized plan of care.
    • Establish patient baseline and needs
    • Communicates with patient families to schedule visits
    • Knowledgeable about state and federal regulations that impact patient care and the provisions of home care practices
  • Demonstrates effective management of the EMR while documenting clinical skills as appropriate in the client record (EMR)
    • Completes and submits all assessments, visit notes, sub-forms, orders, and other pertinent client documents in a timely manner
    • Confirms schedules weekly and as appropriate to ensure accuracy in the frequency of patient visits
    • Responds to office communications within EMR in a timely manner
  • Identifies new problems, changes, and/or the resolution of problems and follows through appropriately:
    • Clinical Supervisor/Team Lead notification
    • Communicates with physicians to address sudden/unexpected changes and/or concerns related to patient status and obtains orders per regulations
    • Documents care provided in clinical notes, physician orders, and coordination of care
    • Demonstrates critical thinking skills
    • Performs specific treatments and medication administration per physician orders
    • Provides support, guidance, and education to patients and caregivers to maintain optimal physical and emotional well-being
    • Communicates with interdisciplinary teams/office/family to ensure the best patient outcomes
  • Participation in Agency’s other mandated activities as requested by Agency Director such as In-services, education, and committee work related to client care, including but not limited to;
    • Participates in training newly hired clinicians in field visits, EMR documentation, and skills competencies.
    • Assists Clinical Supervisor in staff Annual supervisory visits
    • Completes all annual clinical and operational competencies timely
  • Participates in the “After Hours Standby RN” schedule as assigned
    • Available after hours on designated days to assist with admissions or routine/PRN visits
  • Ensures any observed or suspected abuse, neglect, or exploitation is reported to proper authorities as mandated by a health professional

Qualifications
  • Current state license as a Registered Nurse, BSN preferred.
  • 1 year minimum of home health experience and/or pediatric experience is preferred
  • Current state driver’s license, auto insurance as per state law, and reliable transportation
  • Provide copy of current TB results or chest x-ray
  • Current CPR card and Current health certificate as applicable
  • Be flexible, organized, and have the ability to exercise sound judgment to perform the job duties/responsibilities as described

Additional Information


The salary range for this position is $65,000-$75,000 annually.
