Clinical Pharmacist

Duties:

- Collaborate with healthcare professionals to optimize medication therapy and patient outcomes
- Conduct medication reconciliation and MTM services
- Assess medication orders for appropriateness, safety, and efficacy
- Monitor patients' response to medications and adjust therapy as needed
- Educate patients and their families on proper medication use and potential side effects -Provide Diabetes Education - Provide immunizations and POCT Testing
- Document clinical interventions and outcomes in patient records

Skills:
- Strong clinical knowledge and understanding of pharmacotherapy principles
- Ability to interpret laboratory values, diagnostic tests, and medical records
- Proficiency in using pharmacy software systems for order verification and documentation
- Excellent communication skills to effectively interact with healthcare professionals, patients, and their families
- Attention to detail and ability to prioritize tasks in a fast-paced environment
- Strong problem-solving and critical thinking skills - At least 3 years of Clinical Services experience required

Pay: $60.00 - $63.00 per hour

Experience:

• Pharmacy residency: 3 years (Preferred)

License/Certification:

• Registered Pharmacist in New York (Required)
• Licensed Pharmacist in New York (Required)
