The Colorado Coalition for the Homeless seeks a Nurse Manager to manage daily nursing and clinical operations, collaborate with community partners and integrated health teams, support patient and staff needs, and help lead quality improvement efforts for the program. This position is located at 2175 California St., Denver, CO 80204 and reports to the Director of Recuperative Care.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions - Directly supervises all current-state Respite programming including Medical Respite at SSRCC, Park Avenue Inn, and Transitions Care Management.
- Directly supervises RN staff and associated program(s) MA supervisor or MA staff when MA supervisor is not present in program.
- Ensures the quality of care through:
- Oversight of patient-centered integrated treatment plans to direct all services provided, including regular case conferences of all respite patients to discuss admissions, acute and chronic care, and discharge planning.
- Ensuring SSRCC meets Medical Respite national standards and strives for benchmarked best practices in integrated care.
- Monitors program censuses and staff FTE, and schedules team according to ratios and program needs. Ensures the communication and scheduling of staff are adequate for daily needs.
- Participates in and leads quality improvement activities for the Respite program in collaboration with Integrated Health Management Team. Shares knowledge with and coaches other Nursing Services team or inter-disciplinary members on quality improvement & LEAN methodology.
- Maintains close working relationships and collaborative efforts with all internal and external partners including CCH program referral sources; hospitals; shelter partners; and other community-based service providers.
- Develops and/or maintains necessary tools for appropriate documentation of patient care and data collection. Prepares and submits reports to Coalition management and/or funding sources as required or requested. Maintains a medical record system that complies with State, Federal and HIPAA regulations. Works alongside Operations Managers to ensure the quality of SSRCC program registration and practice management system.
- Communicates, organizes, and co-leads discipline meetings on a regular basis in conjunction with other RN Leaders. Facilitates, leads, or participates in other leadership, discipline, or inter-discipline team meetings as appropriate or assigned.
- Maintains a professional and confidential work environment. Help create, coach, and maintain a climate within Respite and CCH Health Services at large that is marked by respect for others; that values inclusiveness and builds workforce diversity; and that fosters cooperation and teamwork.
- Performs direct nursing care and provides patient education in all facilities as needed to ensure adequate staffing levels and quality of care. Maintains competence in all duties outlined in current Registered Nurse job description. Supports, participates in, and covers for other nurse managers and nurse leaders as needed for a cohesive interdepartmental collaboration and team environment.
- Will work with SSRCC leadership team for any workflow creation, improvement, or analysis needed to improve programming.
- Performs other duties as assigned.
Qualifications Summary - Successful completion of an ANA-accredited Registered Nursing program required; Bachelor’s and/or Master’s degree preferred.
- Current licensure as a Registered Nurse in the State of Colorado required.
- Minimum of 3 years cumulative/total nursing experience is required. Minimum of 3 years nursing with experience in a public health, hospital, or community health setting is preferred.
- Prior management/supervisory experience in a healthcare setting strongly preferred. Leadership experience as demonstrated by initiative, team support, program building, etc. in a healthcare setting required.
- Current Basic Life Support certification required.
$86,781 - $104,138 a year
WHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVENT YEARS OF EXPERIENCE
Qualified candidates are eligible for the NURSE Corps Loan Repayment Program which offers payment of 60% of unpaid nursing education debt in exchange for a two-year commitment at a NURSE Corps approved site with an option to extend to a third year for an additional 25% of the original balance.
