Compensation Range: $44.01 - $44.01 / Hour
Department: 875100 Case Management
Expectations: This position involves discharge planning interventions with patients/families under the direction of Case Managers and Social Workers. Under the direction of the clinical staff, the Discharge Planner performs multiple activities that support the movement and transitions of patients out of the inpatient hospital setting, using current knowledge of hospital processes and community services. It involves frequent contact with patients and families, as well as multiple hospital staff and community resource, agency, facility and/or payor representatives. Acts as an internal resource to PFS around discharge planning, serves as a liaison to external case managers, review organizations, insurance companies, around discharge planning. This position requires flexibility of days and hours.
EDUCATION:
Bachelor's Degree in Social Work required or related social field with experience listed below, or Licensed Vocational Nurse with experience listed below.
EXPERIENCE/TRAINING: For BSW: One year experience in a healthcare setting or if a new graduate, field placement in a healthcare setting preferred.
For Bachelor's in a social-related related field: Recent experience of more than three years in case management and/or utilization management preferred.
For LVN: Recent experience of more than three years in case management and/or utilization management preferred.
SKILLS:
Exemplary communication, coordination and problem-solving skills, with emphasis on customer service toward patients, families, community providers and payors.
Type minimum of 40wpm preferred. Strong computer skill Ability to manage activities and multiple priorities within complex systems.
Tolerance for ambiguity, uncertainty and change.
Ability to function in unstructured environment.
