Equitable Social Solutions impacts lives by creating tailored solutions for each person we serve in order to break the cycles of generational poverty and chronic homelessness.

Job Description

  • Effectively communicate both verbally and in writing. Ability to clearly speak to residents, community partners, and other service providers. Ability to demonstrate appropriate and professional email etiquette.
  • Establish effective working relationships with other staff, agencies, partnering organizations, and other service providers.
  • Uphold HIPPA by maintaining confidentiality of files and records.
  • Identify client needs through Strengths and Needs Assessments and develop housing stability plans that document client progress towards housing.
  • Have knowledge of the homeless community and Lane County demographics. Knowledge of Oregon rental practices, fair housing, and local housing resources such as: Lane County Coordinated Entry, Permanent Supportive Housing, Rapid-Rehousing, and other subsidized housing programs.
  • Work with our clients experiencing homelessness to help overcome barriers or issues that affect their ability to obtain and retain housing and move toward self-sufficiency. Support clients with obtaining long-term stable housing. Meet program expectations of housing 1-2 clients per month.
  • Refer clients to appropriate agencies for specialized services and/or counseling. Outsource to local resources when needed.
  • Complete documents and forms, maintain records and files, and schedule appointments
  • Engage in efficient documentation practices such as creating weekly client case notes, documenting appointments, referrals, service transactions, and financial support. This includes documenting all payments for services provided to clients.
  • Engage with clients on a daily basis to support with a variety of needs. This could include de-escalation, harm reduction, and practicing trauma informed care.
  • Utilize crisis intervention to problem solve and de-escalate residents who may experience a variety of co-occurring disorders such as mental health and substance use disabilities.
  • Meet with clients on a weekly basis to address any behavioral challenges, address living space, and support engagement with program policies and expectations.
  • Support with any facility needs such as cleaning, serving meals, or providing basic needs to residents.
  • Support program mission to exceed contractual obligations with Lane County.
  • Other duties assigned

Qualifications

  • Bachelor’s degree in Rehabilitation/Counseling, Education, or Human Service/Allied Health field or the equivalent and
  • Minimum of two years case management and/or related administration experience required
  • Experience in a residential or rehabilitation setting with neurologically impaired clients preferred
  • Demonstrates effective and professional interpersonal, verbal, and written communication skills
  • Strong computer software (Microsoft Office) and data entry skills 
  • Able to work independently and part of a team
  • Organized and able to function under minimal supervision
  • Has reliable transportation and is able to travel to/from multiple assignments/locations on a daily basis

  • Developing immediate solutions to emergency problems and expediting delivery of needed services
  • Documentation of case notes, appointments, referrals, service transactions, etc.
  • Proficient in technology. Utilizing multiple electronic systems such as Outlook, SharePoint, QuickBase, and HMIS.
  • Engaging with people and initiating difficult conversations or utilizing motivational interviewing techniques
  • Trauma informed care, harm reduction, and housing first concepts
