Licensed Clinical Social Worker (LCSW) - Remote IDT
Evergreen Nephrology is seeking a Licensed Clinical Social Worker to provide compassionate, person-centered care for kidney disease patients through collaboration with an interdisciplinary team.

- Manage behavioral health needs including assessment and treatment
- Provide transitions of care support to ensure strong social support
