Nurse Care Manager - RN
Evergreen Nephrology is seeking a Nurse Care Manager to provide holistic kidney care. The ideal candidate is compassionate, experienced in managing patient care, and driven by quality outcomes.
Skills
- Clinical practice experience
- Utilization management experience
- Case management skills
- Knowledge of kidney disease management
Responsibilities
- Build empathetic relationships with patients and families
- Evaluate and identify patient’s needs based on their respective values
- Interface with various healthcare providers on care management plans
- Monitor care and recommend alternative levels of care
- Coordinate patient education and advocate for treatment options
- Prepare reports and communicate program changes
Education
- Current RN License
- BSN preferred
