JOB SUMMARY: The Health Information Coordinator leads by example, maintaining a professional work relationship with the team and residents, and their family members. Remains knowledgeable of residents’ responsibilities and rights, including the right to determine their own Daily Pleasures. The Health Information Coordinator is responsible and held accountable for following Fellowship Senior Living’s mission and core values while maintaining and living our philosophy of person directed lifestyle and Fellowship Spirit.
 
ESSENTIAL FUNCTIONS:
·        Organize / close out resident medical records.
·        Retrieve and gather medical records when requested.
·        Organize Healthcare Center’s medical professional credentials, and ensure documents are up to date and current.
·        Assist in accepting and organizing central supply weekly deliveries, maintaining the central supply area to ensure a clean and neat environment.
·        Be a backup for both Skilled Nursing and Assisted Living scheduling coordinators. Schedules Care Partners (CNAs/CHHAs) and Nurse Partners (RNs/LPNs) for the Health Center adhering to company guidelines and maintaining industry regulations. Makes recommendations regarding staffing assignments as well as any necessary corrective actions. Determine staffing shortfalls and report job vacancies and scheduling problems to human resources and Nursing Administration.
o   With regards to Assisted Living backup
§  Work in the Assisted Living Staffing office on Wednesdays for the full day
§  On Weekends, checks in with the Assisted Living Charge Nurse at the start of shift an and at the 3:00pm change of shift to monitor and address any staffing issues.  Resolutions may include:
·        Posting open slots with agencies
·        Making phone calls to available team members to fill slots
·        Meet with the Assisted Staffing Living staffing coordinator at the end of the day on Tuesday and Thursdays to discuss and be made aware of potential staffing issues
·        Be a point of contact for Assisted Living Nurses for call outs in the absence of the staffing coordinator
·        Backup the Healthcare concierge for lunch
·        Be trained and able to give a tour to the potential new resident/resident family in Assisted Living
·        Perform clerical duties / projects as needed by administration.
·        Other duties as assigned by supervisor or designate

ADDITIONAL RESPONSIBILITIES:
Use confidentiality and proper chain of command when passing along information, changes, or residents concerns.

Maintain a safe, comfortable, and functional home environment by assessing the household for potential hazards, encouraging continuous quality improvement.

Support other team members in the household in meeting all job requirements, actively participate in being a learning and teaching organization.  Willingness to continue to learn and seek improvement.  Attend in-service training programs.

Stay informed of and adapt to changes in the senior living industry, as well as company policies, procedures and best practices.

Accurate, detailed, and dependable, with a commitment to the highest quality standards.
Other duties as assigned. There are times based upon unusual work volume (increase or decrease), emergencies, or severe weather conditions, when it will become necessary for you to accept and perform a work assignment or assist with tasks which are outside of your regular duties or cross-trained assignment in which you possess the basic knowledge and skill to perform the assignment safely.

MINIMUM ACCEPTABLE QUALIFICATIONS:
High School diploma. Associate’s degree and medical record experience, preferred.

The Health Information Coordinator is expected to have acquired the necessary knowledge and skills to perform the job satisfactorily in accordance with current standards, guidelines and regulations of Fellowship Senior Living.

Must have demonstrated leadership qualities, leadership skills, and interpersonal relationship skills.
Possess strong organizational skills. Demonstrate strong communication and relationship building skills and exhibit a friendly demeanor to residents, team members and families.

Excellent written and oral communication skills in English.
 
 
WORKING ENVIRONMENT:
The Health Information Coordinator functions independently, has flexibility, and demonstrates personal integrity when working effectively with the Household team, vendors, and all we care for and serve.  The Health Information Coordinator may be in close contact with individuals who have infectious diseases and may handle potentially harmful chemicals and infectious waste.  The Health Information Coordinator must observe guidelines to guard against disease, accidents, and other dangers, wearing personal protective equipment when needed.  The Health Information Coordinator may be subject to stressful, uncomfortable, and/or emotional situations/individuals.
 

 
PHYSICAL DEMANDS:
The Health Information Coordinator position requires physical activity and moderate movement throughout the workday in order to perform daily duties.  Requires the ability to stand, walk, push, pull, carry, move, bend, kneel, squat, reach, stretch, and lift a minimum of 35 lbs.  The Scheduling Coordinator must use universal precautions in all situations.  In the event there is a need to evacuate the building, heavy lifting may be required to carry residents to safety. 
 
Must be able to operate office equipment (phone, computer, printer, copier, fax machine, etc), and various equipment and mobility devices (wheelchair, walker, gait belt, mechanical lift, etc), used by/for residents.
