About You:- You are an experienced nurse who can quickly and effectively triage and document acute and chronic patient issues, deploy independent nursing interventions when needed and appropriately and clearly escalate clinical issues to the PCP level when indicated.
- You love creative problem solving. As the first triage nurse at Gather, your help will be needed to assist in the development and standardization of triage protocols and incoming and outgoing call workflows. There is a high level of comfort in an ever changing and fast paced Heathcare Start Up setting. You MUST be flexible and open to changes.
- You bring incredible experience and desire more influence in the clinical decision-making process. You have knowledge of value-based care, utilization management and providing care with “At Risk” entities.
- You have experience with and passion for working in at risk/underserved communities, experience with geriatric patients, and the knowledge and skillset to be a trauma informed care provider

- You are willing and able to ask for support and help when needed. Your role will be supervised and supported by PCPs and by Center Medical Directors.
Role Responsibilities:- You are the 1st line of medical telephonic triage when patients call in with clinical issues. You will develop and implement nursing protocols for common presenting issues that can be managed independently with appropriate nursing intervention.
- Attend weekly clinical rounds and offer valuable insight into hospital admissions, patient ED visits. Generate interventions and solutions to help keep patients engaged with Gather so that we can help them meet their healthcare needs while staying safely in the community.
- Balance high volume of care with providing quality attention to patients, making them feel cared for with appropriate documentation in the medical record.
- Independently evaluate and triage common healthcare concerns such as CHF, COPD exacerbations, falls, URIs, UTIs, confusion and deploy resources in the clinic or in the field as indicated. Asks key questions to gain insight into the patient’s clinical condition and able to adjust approach as needed to get necessarily information
- Monitor and interpret remote signal patient data including weights, blood sugars, heart rate, blood pressure. Manage patient call backs, virtual assessments and escalation for concerning patient data.
- Assist with clinical inbox management, prior authorizations, DME procurement and refill requests.
- Additional duties as assigned by leadership
Education, Training & Experience: - Graduate of accredited LPN program
- 4 years of ambulatory care experience, primary or urgent care preferred
- Relevant phone triage experience
- Current AHA BLS Provider certification
- This position is full time on site at Gather Quincy with possible travel needed to other MA Gather sites.
Professional Competencies: - You have excellent communication skills and the ability to explain both complex and simple concepts to patients and families. You are able to listen and comprehend spoken/written word.
- You can communicate effectively with other providers and patients.
- Competency with Athena EMR or willingness and ability to learn. Computer literacy and ability to multitask across multiple platforms
- You can read, write, and communicate in English, additional languages a plus.
- You can be a positive partner on an interprofessional team, and desire to participate and provide input in the training of other providers and support staff.
- You have interest in building longitudinal, trusted relationships with patients/families and desire to be part of a developing model of care with a flexible and resilient attitude and approach.
- You lead, participate and provide input in the training of other clinical and support staff
- You possess strong judgement; capable of deciphering when to follow protocols and when to deviate
- You have the ability to translate complex medical jargon into accessible terms for patients.
- You are a self-starter who takes initiative and can balance multiple and competing priorities
Where You Will Work- Your primary location will be Gather Health in Quincy, MA with some travel required to other Gather Health sites including Lynn, Dorchester and Brockton
What hours will you be expected to work:- Monday- Friday, 8 AM to 5 PM.
