The role you’ll contribute:
Supports the timely and accurate completion of medical records by the medical staff by maintaining a well-organized medical record. Performs duties in support of the hospital mission to ensure the highest quality of patient care in an economically sound and efficient manner. Communicates with physicians / supervisor / coworkers as appropriate to expedite the record completion process.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
- Prepares documents for scanning into the electronic document management system
- Analyzes medical records to confirm patient name, med. rec. #, and account number, checks every page front and back for accuracy and completeness of record and assigns any deficiencies to appropriate clinician for completion utilizing the electronic medical record system
- Examines pages and verifies patient identification on each page
- Retrieves medical records reports and reconciles paper records with departmental documents to verify the receipt of all records
- Maintains computerized record tracking system daily and makes necessary updates and deletions to ensure proper completion of record
- Assists in daily receiving and processing of ancillary reports into the electronic medical record
- Assists and / or provides physician with necessary information / data for the timely completion of records
- Reviews and / or completes medical record requisition and retrieves medical record daily for appropriate internal / external customers
- Demonstrates flexibility in rotating to all / designated areas in the department and functions proficiently in each as observed by supervisor and as evidenced by feedback from others
- Demonstrates ability to assist with internal and external customers' requests for information / reports
- Demonstrates ability to index documents in the correct level as established by facility policy
- Demonstrates ability to minimize duplicate records and overlap entries
- Demonstrates ability to verify data integrity
- Demonstrates ability to review the electronic document management system and records contained within for appropriate image quality and indexing.
Qualifications
KNOWLEDGE AND SKILLS REQUIRED:
EDUCATION AND EXPERIENCE REQUIRED:
- High School diploma or GED
