Care Navigator - Michigan (All Other Counties - Lower Peninsula)

Homeward is a technology-enabled healthcare provider delivering quality, affordable, and comprehensive care to underserved rural communities in Michigan. They are looking for a Care Navigator passionate about improving health inequities and using technology to connect members with necessary services.

Skills

Experience in patient-facing roles
Passion for delivering care in rural America
Expertise with the local community and available resources
Care navigation experience (preferred)
High-growth environment experience (bonus)

Responsibilities

Engage potential members and communicate services provided by Homeward
Build member health literacy and digital literacy
Conduct pre-appointment requirements with patients
Support fulfillment of health services and care plans
Provide coaching for self-care activation
Collaborate with primary care providers for cohesive care
Maintain member panels and community connections

Education

Completion of a Medical Assistant or LPN program
High school diploma or equivalent
