Imagine Pediatrics is a pediatrician-led medical group that aims to improve care for children with complex medical conditions through innovative approaches and virtual care services. They are seeking a Community Health Navigator Manager to supervise social case management services.

Responsibilities

Supervise social case management services
Plan and develop a coordinated approach to services
Oversee department staff to ensure quality and efficiency
Participate in interdepartmental groups for social case management
Establish performance standards and goals
Ensure compliance with ethical and legal standards

Education

Master's Degree in Social Work
Licensed by the State of Texas as a Licensed Master Social Worker (LMSW)
