Community Health Navigator

Imagine Pediatrics is seeking a Community Health Navigator to assist patients in receiving post-hospital care. The ideal candidate is passionate about pediatric care and is able to adapt in a fast-paced environment.

Skills

Excellent communication skills
Strong critical thinking skills
Ability to manage multiple tech platforms

Responsibilities

Assist patients in obtaining post-hospital resources
Coordinate care with facility discharge planners
Work as part of an interdisciplinary team
Support patients with community needs related to care coordination
Liaison with hospitals to raise awareness of programs

Education

Bachelor's degree
Texas or National CHN certification
