Hospice Nurse Case Manager | Hospice Registered Nurse Extraordinaire!
Inspire Hospice and Palliative Care is looking for an experienced RN Case Manager with a passion for providing exceptional care to patients navigating their end-of-life journey.
Skills
- Strong communication and observation skills.
- Understanding of hospice principles and philosophy of care.
- Experience working with interdisciplinary teams.
Responsibilities
- Provide top-notch bedside care to patients and their families.
- Educate patients and families about the disease process and end-of-life care.
- Collaborate with the hospice team to ensure comprehensive care.
- Manage symptoms of patients at the end of life.
- Maintain accurate and timely clinical records.
Education
- Current GA Registered Nurse (RN) licensure.
- At least one year of hospice or home health experience.
