Job Title: Home Health Intake Specialist
Location: Golden Valley, MN
Hours: Full-Time and occasional weekend
Salary: $24 to $28 per hour

Home Health Care Inc is seeking an intake coordinator to join our team here in Golden Valley!You'll get to speak directly with with clients and referral sources and collect information required to start home care services! If you have knowledge of insurance providers and plans, medical documentation, and have an excellent attitude for customer service, we encourage you to apply! Intake Sales Coordinator Qualifications:

• Experience in a healthcare office, ideally home health setting
• Knowledge of medical/healthcare and insurance terminology is highly preferred
• Experience with home care admissions/intake is ideal
• Demonstrated effective verbal and written communication skills
• Comfortable navigating and utilizing electronic medical records and computer programs

Intake Sales Coordinator Responsibilities:

• Review all incoming referrals by conducting an initial screening to ensure they meet the defined admission criteria
• Contact referring locations to gather client medical information and necessary documentation
• Conduct screening and informational interviews with patients and their families as part of the admission process
• Enter client-specific information and communication notes into the electronic medical record system
• Track clients throughout their stay under our care, ensuring their satisfaction, and follow-up as needed
• Maintain a positive presence inside our referral sources' locations
• Consult with families and clients about our services
• Work collaboratively with our clinical staff to assure consistent message to the community and our clients
