Job Description

Devoted Medical Group was founded on the belief that if we treat each member like we would our loved ones, we can meaningfully improve healthcare experiences and outcomes for some of America’s most vulnerable patient populations. The Intensive Home Care (IHC) Program is at the heart of this goal: we are a highly collaborative, multidisciplinary team of physicians, nurse practitioners, case managers, social workers, community health workers, and outreach specialists united by our mission to build a better care delivery system for Devoted’s most complex members. Through a mixture of in-home and virtual interventions, we provide whole-person clinical, social and logistical support for patients with complex chronic conditions, frail elders, and patients at high risk of hospital admissions.
We are currently seeking a Manager of Care Management to manage an awesome team by creating a culture of providing excellent patient care resulting in successful patient outcomes.
The IHC Manager of Care Management is a critical part of the IHC care team, serving as both team quarterback and empowered patient advocate,ensuring the most vulnerable patients are supported, attended to, and enabled to live their best possible lives.

Responsibilities will include:
  • Tracking staff performance through competency check-offs, audits, shadowing, and 1:1 meetings; conducting coaching and providing support as needed
  • Identifying needs, problem solving, and solutioning with the leadership team to find improvements and standardizing the clinical workflow
  • Creating workflows, assessments, and plans of care that create efficiencies for the team and show outcomes for patients
  • Assisting the team in working with escalated patient needs
  • Coordinating and facilitating ongoing education to existing staff and an on-boarding experience for new teammates
  • Partnering with multiple cross-functional teams to drive execution and streamline workflows across programs
  • Meeting with the Senior Manager of IHC regularly to discuss team performance, successes, and areas for improvement

Desired skills and experience:
  • Experience leading clinical teams and operations with accountability for health outcomes
  • Demonstrated ability to drive new initiatives and achieve direct business outcomes
  • Experience managing and developing a high-performing team
  • Strong analytical skills - you are confident making sense of operations through data, you can establish quantitative goals, and you can articulate a case from first principles
  • Strong clinical skills - you are confident in training teams on how to manage complex clinical patients and ensuring that patient needs do not fall through the cracks
  • Comfort with interpreting and acting on data
  • You’re a great team member with a can-do attitude
  • A learning mindset; comfortable operating in a fast moving environment
  • Proven leadership, agility, and collaboration skills
Required Skills and Experience:
  • An active RN or SW license (LMSW+)
  • A minimum of 3 year experience in Case Management
  • Experience managing Interdisciplinary Care Management teams

Salary Range: $104 - 116K annually
