Job Description

This position will support the US Department of Veterans Affairs Critical Staffing Program.  The person in this position will represent International SOS and provide the services outlined below:

International SOS is looking for an individual who is an experienced Ambulatory Care Registered Nurse to provide service for veterans and their families. The primary ambulatory care nurse within the Patient Aligned Care Team (PACT) is responsible for delivering comprehensive and coordinated care to patients in an outpatient setting, focusing on health promotion, disease prevention, and management of chronic conditions. This role emphasizes patient-centered care by fostering strong relationships with patients, families, and multidisciplinary team members to optimize health outcomes and enhance the overall patient experience.

Key Responsibilities

  • Conduct comprehensive assessments of patients, including physical exams, health histories, and needs evaluations.
  • Develop and implement individualized care plans in collaboration with the PACT team, ensuring alignment with patient goals and healthcare objectives.
  • Provide education and counseling to patients and families regarding disease management, preventive care, and healthy lifestyle choices.
  • Coordinate care transitions and referrals to ensure continuity of services and follow-up for complex or chronic health conditions.
  • Monitor and evaluate patient progress regularly, adjusting care plans as needed based on health changes and treatment responses.
  • Document patient encounters accurately and maintain updated medical records to comply with regulatory standards and support quality care initiatives.
  • Collaborate with multidisciplinary teams to facilitate comprehensive care delivery and promote effective communication among team members.
  • Participate in quality improvement efforts, patient safety initiatives, and community outreach programs to enhance health outcomes and service delivery.

Qualifications

Basic Requirements/Certifications:

  • Two (2) years of inpatient care experience.
  • Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e. Puerto Rico) of the U.S. or in the District of Columbia.
  • National Council Licensure Examination for Registered Nurses (NCLEX-RN) to obtain a nursing license.
  • Current Basic Life Support (BLS) certification.

Education Required:

  • Associate Degree in Nursing (ADN) or
  • Bachelor of Science in Nursing (BSN).

Physical Requirements:

  • Work is normally performed in a typical interior/office work environment.
  • Work involves sitting and standing for prolonged periods of time.
  • May require bending, stooping and lifting up to 25 lbs.

Other Special Qualifications:

  • Must be able to read, write, and speak English to effectively communicate.
  • US Citizen or Permanent Resident Card is required.
  • Pass/possess VA Public Trust Clearance.

Offer contingent on position being awarded.

Additional Information

Pay range is based on several factors and may vary in addition to a full range of medical, financial, and/or other benefits. Final salary and offer will be determined by the applicant’s background, experience, skills, internal equity, and alignment with geographical market data. 

Compensation $32/hr-$45/hr

Benefits – Full-time positions are eligible for our comprehensive and competitive benefits package including medical, dental, vision, and basic life insurance.
