JOB DESCRIPTION
INTAKE SUPERVISOR
Description of Responsibilities
The Intake Supervisor is responsible for processing information on new patients as well as for the clinical management and coordination of home infusion services to patients/clients. Also collaborates with the Clinical Pharmacists in resolving patient care issues, and acting as a liaison between home health agencies and case management/utilization review departments. The Intake Supervisor plays a key role in quality assurance monitoring for infusion services.

Responsibilities include the following:
1. Intake information from referral sources and patients in a professional, knowledgeable, and courteous manner and route to the appropriate department or individual(s) in the pharmacy.
2. Establish and maintain accurate and complete patient records.
3. Receive calls from new patients and referral sources, and document all customer communications.
4. Consults with the Pharmacist-In-Charge regarding referrals, medications and cash quotes as appropriate.
5. Assist billing department with verification of insurance coverage for customers.
6. Communicate with billing department to ensure proper billing to insurance companies, contracts, other third-party payers and patients for all products and services delivered; maintain accurate billing records.
7. Communicate with Procurement Supervisor and Pharmacist regarding referrals needing delivery.
8. Coordinates nursing when required and communicates confirmation with appropriate parties.
9. Contributes to ongoing communication with the home health agencies, hospitals, and doctor's offices for the pharmacy.
10. Completing patient admit packet and forwards it to the procurement supervisor.
11. Checks insurance verification is done in a timely matter and entered into the system correctly.
12. Checking to make sure new referrals are logged daily on the "New Referral Log".
13. Executes counseling notices and performance evaluations as needed.
14. Updates patient and nursing contact information in the system as needed.
15. Monitors employee's timesheets
16. Works on-call after hours as scheduled by the Intake Manager
17. Participate in surveys conducted by authorized inspection agencies.
18. Participate in the pharmacy's Performance Improvement program as requested by the Performance Improvement Coordinator.
19. Participate in pharmacy committees when requested.
20. Participate in in-service education programs provided by the pharmacy.
21. Report any misconduct, suspicious or unethical activities to the Compliance Officer.
22. Perform other duties as assigned by manager.
Minimum Qualifications:
1. Organizational skills sufficient to maintain consistently accurate records.
2. Ability to evaluate options and to make efficient decisions.
3. Computer skills that include proficiency with spreadsheets, word processing, and efficient use of the internet and e-mail.
4. Possess and exercise advanced level of accuracy.
5. Ability to use good judgment and work with minimal direction.
6. Must possess excellent oral and written communication skills, with the ability to express technical issues in "layman" terms.
Education and/or Experience:
Must have a High School diploma or Graduation Equivalent Diploma (G.E.D.) Prior experience in a pharmacy or home health company is of benefit. Prior experience in a consumer related business is also of benefit.
