RPM Specialist

• Monitoring of assigned patient load patient’s vital signs.
• Communicating and establishing a relationship with the patient, their providers, and family members about the patients care and health
• Monitoring and responding to crisis alerts in a timely manner by communicating with patients and providers to assist in managing the patient’s care.
• Communicating with doctors, patients, and family members about patient health.
• Providing a copy of the patient’s vital signs to the providers involved in the patient’s care as needed and appropriate.
• Communicating patient’s needs to the appropriate care team members and following up accordingly.
• Educate patients in the use of monitoring equipment and proper processes to use to ensure accurate information is being transmitted.
• Managing patient’s data and medical records as confidential records following HIPAA guidelines.
• Maintain the patient’s comprehensive clinical record through detailed documentation in EMR.
• Must be bilingual

Job Type: Full-time

Pay: $15.00 - $16.00 per hour
