What You’ll Do:

- Conduct assessments of patients and families to identify home care needs

- Perform physical evaluations and document medical history

- Develop, initiate, and adapt care plans with family involvement

- Prepare discharge plans in advance

- Educate and support patients and families on ongoing care needs

Requirements:

- RN license from an accredited nursing program

- Minimum 1 year of recent RN/LVN/LPN experience, preferably in home health, public health, or hospice

- Active state licensure and CPR/BLS certification

- Valid driver’s license, insured vehicle

- Strong communication, problem-solving, and clinical skills
