Care Management Consultant - Remote
NeueHealth is a healthcare company focused on creating a consumer-centric care experience. We are looking for a Care Management Consultant to promote quality and cost-effective outcomes for a population.
Skills
- Working knowledge of Case Management and Quality Management practices
- Risk Adjustment knowledge
- Microsoft Applications: Word, Excel, and Powerpoint
- Visio knowledge preferred
Responsibilities
- Conduct clinical audits to ensure alignment with clinical standards
- Collect and analyze data from Care Partners to assess outcome and operational metrics
- Prepare and conduct patient-level monthly clinical performance meetings
- Collaborate with clinical leaders to assess process improvement initiatives
- Coordinate special clinical assurance projects related to clinical management
Education
- Associate’s degree in Nursing preferred
