POSITION TITLE: Intake Coordinator (Patient Services)

REPORTS TO (TITLE): System Director of Patient Services

DESCRIPTION OF POSITION:

To assess the needs of people seeking an assessment and ensure their referral to the service or resource that best addresses those needs. To link recipients to appropriate resources or care providers. To evaluate based on Admission Criteria and Medical Necessity. To provide a smooth compassionate customer service experience for patients and their families.

CURRENT STAFFING NEEDS:

FT/ Intake Coordinator (Tuesday-Saturday 3:00PM - 11:30PM)

KEY RESPONSIBILITIES:

  • Receive service requests from multiple community sources. These come in the form of e-mail, fax and telephone. Demonstrates ability to establish rapport with these prospective patients. Meet and greet upon admission.
  • Effectively evaluates referral packets from emergency departments, Crisis Teams, and private practices. Demonstrates ability to review these packets for medical necessity and appropriateness of admission criteria for our facility and/or referral to an outside source.
  • Conduct screening via telephone or tele-video. Ensure that potential patient, family and referral sources’ needs are met.
  • Document calls and related information on Assessment form and enter date into Assessment system. Demonstrates ability to work well under pressure and multi task.
  • Stay abreast of organization's programs and services. Obtain and document all pertinent information needed for level of care determination.
  • Supports the department’s philosophy for customer service. Must be able to effectively communicate internally and externally in a professional manner at all times, this includes all electronic, phone, face to face, and mail communication. Communication must be polite, clear, concise, without abbreviations/jargon, and without mistakes.
  • Establish and maintain positive relationships with physicians, allied health professionals, and all referral sources. Demonstrates ability to communicate (verbal and written) effectively and maintains regular communication with supervisor and co-workers.
  • Participates in day-to-day activities and operations of the department. Participates in activities which enhance professional growth and development to include clinical oversight requirements.
  • Performs related duties, as requested.

Education/Licensure

  • Bachelor’s Degree in Psychology or related field.
  • Must be 21 years of age.
  • Basic knowledge of, and skills in applying principles of abnormal psychology and mental health problems; familiarity with psychological, sociological and medical terminologies; skills in analyzing and evaluating information; problem solving skills; excellent oral communication skills; legible handwriting, ability to concentrate on tasks and meet deadlines; ability to confidentially maintain all information; persuasive skills with prospective patients and family members who demonstrate denial or resistance to treatment.

  • Makes customers/patients and their needs a primary focus. Dedicated to meeting the expectations of internal and external customers/patients. Represents the organization in a positive and professional manner. Solicits feedback and uses it for improvement in service.
  • Maintain a working knowledge of commonly used policies, awareness of policy changes and know where to find additional polices and needed information.

Physical Requirements:

While performing the duties of this job, this position is frequently required to do the following:

  • Stoop, kneel, crouch, reach, and stand for sustained period of time.
  • Walk, push, pull, lift, carry objects from a lower to higher position or horizontally from position to position or otherwise move objects.
  • Feel sizes, shapes, temperatures, and textures by touching with skin, particularly that of the fingertips.
  • Express or exchange ideas orally and potentially loudly, accurately, or quickly.
  • Perceive the nature of sound with no less than a 40 dB loss @ 1000 Hz and 2000 Hz with or without correction.
  • Exert up to 100 pounds of force occasionally, and/or up to 20 pounds of force frequently.
  • Able to provide manual holds for patients who are physically acting out and are potentially dangerous (at times this occurs on the floor.)
  • Minimum standard of visual acuity with or without correction.
  • While worker may possibly be subjected to temperature changes, the worker is generally not substantially exposed to adverse environmental conditions as the work is predominantly inside.
